Allied and Community Health Services
District NursingOur District Nursing Service provides holistic support, caring for patients within their own own environment, maintaining independence with dignity. We provide a seven day a week service - Monday to Friday 8.00am – 4.30pm and on weekends and public holidays reduced hours. After hours needs are met on an individual basis. |

The service includes: • General nursing, personal care and hygiene assistance • Care and support following discharge from hospital • Wound and ulcer care • Medication management, including intravenous therapy • Palliative care • Bowel and bladder care, incontinence advice • Community (Homebased) Nursing service for ACC patients • Ostomy, diabetes, cardiac advice and follow-up • Assessments for domestic assistance and meals on wheels • Support services for chemotherapy patients • Supervision of oxygen therapy, respiratory conditions • Health education and support • Assessment of health needs • Liaison with other health professionals for continuity in care • Short-term loan equipment eg bed cradles, commodes • Referrals can be made from your GP, hospital, or other

Meals on WheelsGore Health is contracted to supply Meals on Wheels to recipients in the Gore, Wyndham, Mataura and Edendale areas. We are subsidised by the Southern District Health Board and at present we are allocated approximately 11,800 meals a year. The meals consist of a cooked two-course midday meal, Monday to Friday. The meals are delivered by volunteer drivers.
To receive Meals on Wheels you have to be first referred by a GP or other health professional and then have an assessment of suitability completed by a District Nurse. If you qualify for Meals on Wheels the District Nurse will allocate a certain number of meals each week. Dietary needs are taken into account when preparing meals.

Occupational TherapyThe Occupational Therapy Department is open from 8.30am until 5.00pm, Monday to Friday. When disability, illness or injury limits a person's ability to function, an Occupational Therapist can assist them to achieve and maintain their maximum level of independence and safety. At Gore Hospital we offer the following inpatient and community Occupational Therapy services:
• Activities of daily living retraining • Provision of adaptive equipment, including wheelchair and seating • Housing modifications • Cognitive assessments • Concussion follow up • Pre-operative joint replacement home visits (Mercy & Dunedin Hospitals)

PhysiotherapyWe contract Functional Physiotherapy to provide inpatient and community Physiotherapy services for Gore Hospital. Physiotherapists work in partnership with individuals to develop, maintain and restore maximum movement and physical function. They assess, diagnose, and treat movement disorders arising from injury and disease. Physiotherapy services at Gore Hospital include:
• Inpatient Physiotherapy to patients in our Medical, Assessment Treatment & Rehabilitation and Maternity wards • Community Physiotherapy for those in their own home to address issues around transfers, mobility, balance, • Mobility Assessments and the issuing of short term loan and long term walking aids • Outpatient clinic for joint injuries, back and neck pain and neurological disorders. Needs AssessmentGore Health, in conjunction with the Southern District Health Board, provides a Needs Assessment Service to support older people or those with Age Related Needs to manage as independently as possible within their own environment. The assessment process covers the client's physical, mental social and emotional wellbeing taking into consideration the clients wishes and goals. From this we determine how a client can best be supported to maintain their independence and how they can be assisted with tasks that may be getting too difficult. A Needs Assessment could be carried out to see if a client is eligible for the following - domestic assistance to help with household tasks, personal cares, bathing, showering, dressing. meals on wheels or meal preparation, day care, respite care, rest home placement. This Service is available to any person with an age-related condition, which may continue for 3 months or longer. Referrals can be from a Doctor, District Nurse, Healthcare provider or by phoning to speak with the Needs Assessor. The Needs Assessment itself takes about one to two hours and involves a general discussion with the client. A friend or family member is welcome to offer support during this discussion. Once the Needs Assessment is completed it is sent on to Service Co-ordination at Southland Hospital. A Co-ordinator will determine if the client qualifies for funded services and arranges a suitable care plan.

Social WorkSocial Workers support individuals, families, whanau, communities to manage everyday living and make lifestyle choices that achieve quality of life and enable/empower people to manage their own lives/living conditions. Social Work Services at Gore Health compliment services provided by other healthcare professionals. Tasks undertaken by our Social Worker include:
• Provision of support to both in/out patients • Assisting people with issues around Benefit entitlements and making applications for Benefits eg. Disability Allowance • Linking people with other healthcare professionals • Organising Carer Support to enable a fulltime caregiver to have time out • Assisting people to make decisions on future accommodation needs by outlining options/choices eg Resthome, • Informing of possible attendance at Resthome Day Programmes and other community based activity progammes • Assisting people to achieve funding for travel/ accommodation when required to travel away to another area for treatment. • Providing information on safety alarms, and other equipment that may be required • Working in partnership with GP's, practice nurses and other community care providers • Linking people with community friendship networks • Assisting people with relationship management, grief counselling, facilitation of family meetings • Advocating for the rights of people at both a local and National level.

DietiticsAt Gore Hospital our Dietician provides Dietitic services for our inpatients and outpatients on a part-time basis. This includes nutritional assessment and education. Outpatient clinics are held on a Wednesday - 9.30am to 5.00pm - and also on some Tuesdays. Referrals can be made by either writing to the Dietician Gore Hospital P O Box 274 Gore 9740 or by phoning.

Speech Language TherapySpeech Language Therapy services at Gore Hospital are provided by Meryl Jones, Speech Language Therapist on a part-time basis. She looks after our inpatients who have communication problems and also runs outpatient clinics on a regular basis. She works closely with people who have communication disorders and their families, whanau and others. This includes the identification, remediation and prevention of problems interfering with effective swallowing and communication.
