Gore Hospital has 16 inpatient beds for acute medical and assessment treatment and rehabilitation patients. We have around 800 admissions a year to our inpatient area.
Our medical admissions include people with respiratory and cardiac disorders and patients with multiple medical conditions.
The Assessment Treatment & Rehabilitation area of the ward aims to improve the health, function, well-being and independence of people with disabilities. These conditions include stroke CVA, confusion and dementia, frail elderly, osteoporosis, osteoarthritis, and Parkinson’s disease.
Patients are referred to the A T & R unit of the ward through their GP, Specialist, or other hospital. Assessments are made of these people by our qualified assessment team, appropriate treatment is given and they are rehabilitated back into the community or into a residential facility (e.g. Rest home). Patients are encouraged to achieve as much independence as possible.
Our multi-disciplinary team consisting of :
work together with our patients to achieve the best possible health outcome. They ensure our patients receive the necessary treatment and rehabilitation interventions and consult fully with patients/family/whanau to seek agreement on services to be provided. Prior to our patients discharge the team meets to discuss/agree on the level of support required by the patient at home. Support services are then co-ordinated through members of the multi-disciplinary team.
We believe our patients are should be treated with dignity and respect and be involved actively in decision making and treatment while in the hospital. Family/Whanau/significant others are encouraged to also participate in decision making and other aspects of their care.
