Comprehensive Primary & Community Team (CPCT)
Healthcare That Meets People Where They Are
The Hokonui Comprehensive Primary and Community Team (CPCT) is a locally designed initiative improving access to healthcare across the wider Gore District.
Managed by Gore Health and delivered in partnership with Gore Medical Centre and Mataura Medical Centre, CPCT extends primary care beyond the clinic through a flexible, mobile and culturally responsive model of care.
Working alongside patients and whānau in their homes and communities, the team provides practical support that promotes wellbeing, reduces health risks and helps people access the care they need when they need it most.
How We Help
CPCT brings together a small, integrated team of clinical and non-clinical professionals who provide short-term support tailored to each person's needs.
Services include:

Health system navigation

Connecting people and whānau with health and community services

Support to attend healthcare appointments

Assistance with barriers such as mobility, living conditions and access to care
By building trusted relationships, the team helps prevent people from falling through the cracks of an increasingly complex health system while reducing unnecessary demand on General Practice and Emergency Department services.
Making a Difference
Developed through the Hokonui Community-Led Health Programme, CPCT has quickly become an important part of healthcare delivery across Eastern Southland.
Since launching in mid-2024, the programme has:

Received more than 360 referrals

Delivered over 1,300 patient and non-patient contacts since July 2025

Received 114 new referrals while successfully discharging 163 patients, demonstrating an effective short-term intervention model

Provided approximately 25% of patient contacts to Māori and Pacific peoples

Built strong referral pathways and growing confidence among local healthcare providers
Improving Health Together
CPCT demonstrates the value of healthcare designed by the community, for the community.
By combining clinical expertise with practical support and strong local partnerships, the programme is improving health outcomes for people with complex needs while creating a more connected and sustainable healthcare system across the Gore District.
