A Needs Assessment helps identify the care and support services that may assist you or a family member to continue living safely and independently. Gore Health works in partnership with Health NZ to assess individual needs and connect eligible people with the services and resources that best support their health and wellbeing.
Our team takes the time to understand your circumstances and works with you, your whānau and healthcare providers to develop a plan tailored to your individual needs.
Who Can Be Referred?
A Needs Assessment may be arranged following a referral from:

▸ A District Nurse
▸ Another healthcare provider
